Insurance and Payment Policy

At Reno Sparks MedSpa, Dr. Van Reken accepts most insurance plans. When calling for an appointment be sure to give us your insurance info so we can verify your benefits. Some insurance companies such as HHP rrequire that you list Dr. Van Reken as your primary care doctor. They require YOU to change this primary care doctor listing on your own with them prior to your visit with us, usually by phone or online. We are not allowed to do this for you. If you fail to do so they usually consider you uninsured for that visit and require that you pay the entire fee yourself.

When arriving for your first visit please provide us with your ID and your current insurance card(s). Copies will be made and put in your file.

We are participating providers with most insurance companies and accept the following plans:

• Hometown Health
• CDS
• United Health Care
• Blue Cross Blue Shield
• Blue Cross Blue Shield of California
• PEBP
• Health Plan Of Nevada
• Universal Health

This list will change from time to time. Be sure to ask when you book your appointment if we are accepting the insurance you’ll have on the date of your appointment.

If you are uninsured we do accept cash, check, Visa, Mastercard and American Express. We accept both credit and debit cards. Payment is due in full at the time of service. You are also welcome to pay for co pays and/or noncovered services, such as cosmetic services, with cash, check or credit card.

No Call No Show Fee

Patients must call 24 hours in advance or will be charged a fee:

• Medical Appointments for Established Patients – $150
• Medical Appointments for New Patients – $350
• Cosmetic Appointments – $50

Cancellation Policy

If you are unable to keep an appointment, we ask that you kindly provide us with at least 1 full business day’s notice. Your courtesy will make it possible for us to give your appointment to another patient. There is a cancellation fee for medical visits and sometimes for cosmetic services. This depends on written contracts with us for return visits and oral contracts for those who are new to the practice. The charge may depend on the time budgeted out of our schedule to provide the service.
