Cosmetic Service Monthly Membership Information

What services are included:

cosmetic services such as facials, laser services, vaginal rejuvenation, radiofrequency services, PRP for cosmetic reasons, cosmetic or cosmeceutical products and medical services not covered by insurance companies such as PRP injections for orthopedics problems or for sexual health in men or women.

How services are provided:

1st month is prorated on basis of a 30 day month. Example: Your purchase of a platinum 500/month membership is made on the 11th. You pay for 19 days of that month (30-11) which is 19/30 x 500 or 316.67, for the first month, then 500/month.

Cancellation:

You may cancel for any reason at any time, in writing via email or letter, with 60 day notice. Example: you would like to cancel your 500/month membership as you are moving to Hawaii. We are notified of your cancellation via email or letter on 11/15/19. Your cancellation of payments is effective 60 days later, on 1/15/20. Therefore, your last payment is 1/1/20. You may, if you wish, convert any or all of your banked dollars into gift cards for yourself, friends or family.

Gift Card option:

Christmas is coming, and you have some gifts you would like to give to relatives and friends. You have a platinum membership for 500/month. However, it is now December 5th and you only spent $250 last month so you have 250 in the bank from November plus the 500 you just paid for December– 750 in the bank. You decide to get a 350 service and get four 100 gift cards for 4 different relatives.

How are services rendered:

An example is you buy a platinum Gold package for 375/month on June 1st. HydraFacials at that time cost 160 and with your gold membership 7.5% discount they cost you 148 and you get one on the 15th of June and the 15th of July, using up 296 of your 750 banked dollars, leaving 454. On June 25th you want a 400 service and buy 200 of product. With your 7.5% discount your service costs you 370 and your product with your 12% discount costs you 176, the total being 546. Since you had 454 in the bank you now pay us 92. You can always get services sooner than your monthly membership draw will allow by paying for them.

Pregnancy and breast feeding:

If we are notified in writing that you are pregnant and/or breast feeding we will defer all payments until you deliver and/or stop breast feeding.

Services are available on and/ after 5/1/19
We are open 9am – 3pm each 2nd Saturday of the month
